Social work is a profession like others such as medicine and nursing where altruism plays a role in the motivation of its professionals. While altruism is admirable and desirable, it can also be complicated when it involves boundary issues and other ethical challenges. This month’s E-News Exclusive by ethics expert Frederic G. Reamer, PhD, addresses several of those issues with thoughtful discussion of case examples and guidelines for social workers.

Field Notes highlights a report that finds the majority of drug-abusing offenders are not getting the treatment they need in the criminal justice system and the start of new research that will study which emergency treatment is best for children who suffer severe, prolonged seizures that can be life threatening.

The Challenge of Altruism

One of the great privileges of my social work career is the opportunity to work closely with remarkably dedicated, compassionate, and altruistic colleagues. I am continually awed and inspired by social workers’ persistent determination to give of themselves to help others, often in the face of less-than-ideal working conditions, organizational support, and salaries. For many—perhaps most—social workers, professional service is a secular calling in the most virtuous sense of the term.

Most social workers manage their altruistic instincts skillfully. They become involved in clients’ lives with a clear sense of professional boundaries. On occasion, however, social workers’ wish to help others produces daunting boundary challenges.

Offenders Not Getting Needed Treatment in Justice System

The vast majority of prisoners who could benefit from drug abuse treatment do not receive it, despite two decades of research that demonstrate its effectiveness, according to researchers at the National Institute on Drug Abuse (NIDA). In a report published in The Journal of the American Medical Association, NIDA scientists note that about one half of all prisoners (including some sentenced for non­–drug-related offenses) are dependent on drugs, yet less than 20% of inmates suffering from drug abuse or dependence receive formal treatment.

“Treating drug-abusing offenders improves public health and safety,” says Nora D. Volkow , MD, NIDA director and a report coauthor. “In addition to the devastating social consequences for individuals and their families, drug abuse exacts serious health effects, including increased risk for infectious diseases, such as HIV and hepatitis C, and treatment for addiction can help prevent their spread. Providing drug abusers with treatment also makes it less likely that these abusers will return to the criminal justice system.”
